Do hereby acknowledge the following:
1. I am in good health and there is nothing which renders me unfit to undertake this safari trip I am familiar with and will obey all the rules and regulations spoken and written and established for Millennium Tours and Travel, its associates and agents.
2. I agree to Millennium Tours and Travel, its associates and agents taking my photographs which may be used for marketing campaigns, advertising and administrative purposes.
3. I freely accept and assume full responsibility for all risks and possibilities of personal injury, death, damage and loss of property which may arise during the safari and I am also aware that medical services or facilities may not be readily available during the time I am participating in this Safari.
4. I acknowledge that I am fully aware of the dangers and risks that may occur during the Safari arising from the presence of wild animals, incidences and accidents and the risks of suffering bodily harm, death, injury, or loss of property and therefore agree that in the event I am injured, or my property is damaged I will bring no claim, legal or otherwise against Millennium Tours & Travel, its associates and agents.
5. I confirm that I have carefully read and understand this waiver in its entirety. I understand that this agreement represents the entire agreement between myself and Millennium Tours & Travel, agents, associates and that it is binding on myself and my Legal Representatives.
